The CFR doesn't pretend to know how many actual cases there might be. That would be the IFR. The IFR, though, is anybody's guess since the number of infected people is also a guess - except in a subsystem like the Diamond Princess or the Life Care Center of Kirkland, Washington in which case it is the same as the CFR. Across the world, the IFR will never be known.

But the CFR will and it's worth discussing, particularly when there's an interest in comparing COVID-19 to other diseases (that also have unknown IFRs.)
