A trial runs for some period of time - say, 180 days. At the end of that time, they tally up the numbers to see whether those who received their medicine fared better than those who didn't.

I understand the concept. My Dad participated in a medicinal trial years ago. Another nightmare.

That's called a double blind study, everything randomized and nobody knowing what's what.

SOUNDS LIKE OUR GOVERNMENT.

Now, if they TOLD their participants whether they'd received the real medicine, the ones who didn't might behave differently than the others, perhaps not going out to eat as often or reducing their interactions with hookers due to concern that they'd better be careful. That could affect the results, making those who had only had a placebo look better than they would otherwise, and spoil the whole study. Statistically, neither group should know their own status until the entire thing is over.

Probably, even the Merck experimenters didn't know the status of their patients until the end of the trial (which might still be underway, so far as I know.) That's called a double blind study, everything randomized and nobody knowing what's what. It's the only way to be sure that nothing biases the outcome.
