Orthopedic surgeons and physical therapists are the unashamed sadists of the medical profession. They have absolutely no remorse about inflicting pain and laughing about it.
Back in the eighties, I had a bone spur that had grown to an extent that it was interfering with the ligaments when I bent my right knee (the one I just had replaced). Insurance wanted me to get a second opinion before they okayed surgery to take it off, so my orthopedic surgeon sent me downstairs to another orthopedic surgeon who was a friend of his. That guy took one look at the x-ray, had me sit down on the edge of a table with my legs dangling, pressed his thumb hard on the tip of the spur, and told me to raise my leg. DAMN! He got a big ol' laugh out of it and said, "Hurts, don't it?" He then immediately recommended the surgery.
And now, my friends from the Charles Manson Institute of Physical Therapy are happily working me over - all in the name of "bettering" my right knee's functions. So far, the only "function" they've improved is its ability to feel pain.
